TEAMcare

The most difficult and costly patients often have multiple comorbid conditions. Care for these patients with complex conditions is compounded by psychological and behavioral impairment—such as depression, unhealthy lifestyles, and poor adherence to treatment. Research has shown a bidirectional link between depression, diabetes, and coronary heart disease. Additionally, only 50% of patients with depression and diabetes/coronary heart disease have their depression accurately diagnosed in primary care.

TEAMcare is a highly effective program that improves disease control in patients with diabetes and/or cardiovascular disease who also have depression.

The TEAMcare program:
  • Focuses on teaching patients self-care skills to control illnesses
  • Is delivered in the patient’s primary care clinic and by phone
  • Takes a team-based approach
  • Increases behaviors that enhance quality of life

TEAMcare is an evidence-based, patient-centered approach that enhances the primary care team’s ability to provide optimal care. Using the Chronic Care and Collaborative Depression Care models, TEAMcare provides comprehensive care for diabetes, coronary heart disease and depression simultaneously. Compared to usual care, the TEAMcare intervention significantly improved both medical disease control and depression.*

Cancellation Policy:

If you register and are unable to attend, you can apply your Fee toward a future training within one year of University of Washington's receipt of cancellation notification. (Costs incurred by the registrant, such as airfare and hotel, are the responsibility of the registrant.) The University of Washington reserves the right to cancel this course twenty (20) days prior to the course date when a minimum enrollment is not met or for other reasons that prevent the University from holding the course. Each registrant will be notified by telephone followed by written notification and a full refund.

TEAMcare is one of the many software programs and tools created at the University of Washington available through our Express Licensing Program. These agreements have standard terms and conditions that allow for rapid licensing by companies, organizations, or individuals.


The conditions and financial terms of this license apply only to those who download the license, sign it and return it "as is" to UW CoMotion. Some Express Licenses may be modified for an additional cost. Other licensing arrangements may be available.
